  • I D card copy.
  • 7 Months bank statement. ( Approx 10000 $ balance in pkr )
  • Account maintenance certificate.
  • Local company letter head.
  • National tax number.
  • Chamber of commerce certificate.
  • Last 3 years returns.
  • Company invitation letter from Indonesia. ( Color copy or original )
  • Polio Certificate.
  • Confirm hotel booking.
